Services we provide
Assist with daily living
At Aberstone we can support you through our daily living services which are designed to empower you to achieve your goals and dreams by helping support your independence. These include:

Domestic assistance: Support to keep your home clean and tidy, for daily life wellbeing, and to help you maintain your tenancy. This includes cleaning, dusting, dish washing, bed making and taking care of clothes including washing, ironing, folding and hanging.

Meal preparation: Support in planning the meals, budgeting, shopping, cooking, cooking techniques, and general safety in the kitchen.

Independent living skills: Support with how to manage money, pay bills, use a calendar to book appointments, and join clubs or activities.

Personal care: Support with showering, hair washing, toileting, using personal equipment, pressure stockings.

Gardening and yard maintenance: Support to maintain the garden and yard including basic household jobs such as lawn mowing, pruning, cleaning pathways, general tidy up, changing lightbulbs, fixing dripping taps and easing that sticking door. Assistance can be provided for small repairs and maintenance on a case by case basis.

Support and Transport: Support with how to use public transport. We can also help with transport to appointments, meetings and activities using your NDIS transport allowance, or we can invoice for the service.

Social Support

Group activities: Our groups deliver a range of programs including activities such as music, bingo, arts and crafts, cinema, and outings in the community.

Individual social support: Support to access activities of your choice in your community for your overall wellbeing such as swimming, gym workouts, music and dance, arts and crafts, and Riding for the Disabled (RDA). If you really want to challenge yourself, we have even organised for clients to attend a shooting range, get behind the wheel of a specialised Go Kart, or take a leap of faith with a parachute jump.
Recovery Coaches
Aberstone Recovery Coaches work with you to develop recovery-enabling relationships. This includes:

Supporting participants’ engagement with the NDIS.

Coaching to increase recovery skills and personal capacity including motivation, strengths, resilience, and decision-making.

Managing the recovery planning for participants.

Collaborating with the broader system of supports.

Recovery Coaches support individuals by working with the competencies set out by the Australian Health Minister’s Advisory Council National Framework for Recovery-Orientated Mental Health Services.

Aberstone will work with you as we know that sometimes you need a support to get through a crisis and get on with your life. This support has been created in response to a recovery-based approach. Our team looks forward to continuing to assist both our existing participants and new participants, in order to build their capacity and work collaboratively with you and your families to implement your recovery plans.
SDA
Specialist Disability Accommodation (SDA) refers to accommodation for people who require specialist housing solutions to assist with the delivery of supports that cater for their extreme functional impairment or very high support needs.
SDA funding under the NDIS will stimulate investment in the development of new high-quality dwellings for use by eligible NDIS participants. SDA is not funding for actual support services; it is funding for the homes in which these services are delivered.

Medication management

Overseen by our clinical team, we offer Medication Management, with support workers who are fully trained to a national standard, to support you to take medication at the correct time daily. We also work with Participants who have Community Treatment Orders.
Nursing
Our Aberstone community nursing team delivers disability related health supports in conjunction with other allied healthcare professionals. We can support you with a range of services that allow you to remain in your home and minimise the need to attend hospitals or Doctors’ surgeries.

The health system will continue to be responsible for the diagnosis and clinical treatment of ongoing and chronic health conditions not related to a participant’s disability, time limited (non-ongoing) palliative conditions, and acute/post-acute care (e.g. hospital and hospital in the home).

Aberstone’s inclusion of disability-related health supports within an NDIS funding packages provides an opportunity for health supports to be built into your plan.

Aberstone’s clinical team can support your complex needs through our high intensity daily personal activities, and are happy to discuss your individual support requirements.
